Function-4 has released a new guide to cyber insurance requirements, intended to help businesses understand which security controls insurers now expect applicants to prove before approving or renewing a policy.
-- Texas-managed IT provider Function-4 has published a new guide to the cyber insurance requirements local businesses are likely to face in 2026 and beyond, examining what underwriters are asking on applications and explaining how organizations can prepare for their next policy renewal.

The guide is aimed at small and midsize businesses that complete insurance questionnaires without an internal security team. Function-4 notes that insurers now ask applicants to prove their controls rather than simply confirm them, a shift driven by rising losses. As an example, Verizon's 2026 Data Breach Investigations Report, cited in the guide, found ransomware in 48% of breaches, up from 44% a year earlier.
At the core of the guide is a checklist of seven areas most carriers ask about. These include multifactor authentication on key accounts, endpoint detection and response on nearly every device, and protected backups that are regularly tested. Patch management, email security and training, privileged access controls, and a written incident response plan complete the list.
The guide also explains how insurers verify answers, with underwriters commonly requesting dated coverage reports, patch records, and restore test results. It outlines the possible outcomes of falling short, from conditional quotes to ransomware sub-limits and higher premiums, and recommends starting preparations about 90 days before renewal so there is time to close any gaps.
"Buying the policy is rarely the hardest part for our clients," a Function4 spokesperson explains. "The bigger challenge is keeping every attested control running all year. If an investigation after a breach shows a control was missing, the claim can be challenged, so we wanted to set out clearly what insurers expect from local businesses."
